Adult Companion Services

Non-medical care, supervision, and socialization provided to an adult (age 21 or older).

Career Preparation Services

Services aimed at preparing participants for careers through exposure to and experience with various careers and through teaching such concepts as compliance, attendance, task completion, problem solving, safety, self-determination, and self-advocacy.

Case Management

Case Management Services are provided to people who are eligible for DDSN services to assist them to access a full array of community services including medical, social, educational and other needed services that are effective, cost efficient and necessary to avoid costly residential placement thereby making it possible for people to reside in their own homes and communities.

Community Services

Services aimed at developing one's awareness of, interaction with and/or participation in his/her community through accrual of social capital.

Community Services - Individual

Services aimed at developing one's awareness of, interaction with and/or participation in his/her community through accrual of social capital.

Day Activity Services

Activities and services provided in therapeutic settings to enable participants to achieve, maintain, improve or decelerate the loss of personal care, social or adaptive skills.

Early Intervention

Early Intervention Services are therapeutic, training and support services that facilitate the developmental progress of children whose developmental patterns are atypical due to the influence of certain biological and/or environmental factors. Early Intervention includes developmental assessments, treatment planning, home visits and supports to enhance the development of the child and support his or her family in their care of the child.

Employment Services - Group

Intensive, on-going supports for participants for whom competitive employment at or above minimum wage is unlikely.

Enclave - A small group of people (usually 8 or less) with developmental disabilities, who work under the supervision of an employee of the provider agency, in a community business/industry along side non-disabled employees to produce goods or services controlled by the community business/industry (ex. janitorial services at a specific business/industry etc.). The contractual relationship is between the business/industry and the provider agency, whereby the provider agency then pays the individual.

Mobile Work Crew - A small group of people (usually 8 or less) with developmental disabilities, who work under the supervision of an employee of the provider agency, as a self-contained business who typically move to different work sites, by selling a service (ex. landscaping, janitorial) to purchasers within the community excluding provider agencies. The contractual relationship is between the business/industry and the provider agency, whereby the provider agency then pays the individual.

Employment Services - Individual

Intensive, on-going supports for participants for whom competitive employment at or above minimum wage is unlikely. Assessment, job development, placement, and training involve direct facilitation and instruction by DDSN job coach staff. Individual community placement provides support in; community based instruction, career awareness, skills acquisition, strategic on the job training, long term support and follow-along. Ongoing supports and identification of long term natural supports are imperative for the person with significant disabilities to participate in competitive employment and to ensure job stabilization without support throughout the tenure of the placement.

Enhanced Nursing Services

Enhanced Nursing Services - includes both LPN and RN.

Environmental Modification Assessments

Assessment of physical adaptations to the participant's home that are necessary to ensure the health, welfare and safety of the participant

Environmental Modifications

Physical adaptations to the participant's home which are necessary to ensure the health, welfare and safety of the participant (e.g. installation of ramps and grab-bars, widening of doorways, modification of bathroom facilities, etc.).

ICF/ID - Community

An ICF/ID is a facility licensed for the primary purpose of providing health or habilitative services to people with intellectual or related disabilities who require the aggressive, consistent implementation of a program of specialized and generic training, treatment and health services.

Intake

Intake is defined as the collecting and submission of an accurate and complete set of documents in order for DDSN to determine if the service user is eligible for DDSN's services.

Private Vehicle Modification Assessment/Consultation

Private Vehicle Modification Assessment/Consultation- assessment of the specific modifications/equipment needed, any follow-up inspection after modifications are completed, training in use of equipment, repairs not covered by warranty, and replacement of parts or equipment.

Private Vehicle Modifications

Modifications to a privately owned vehicle used to transport the participant (e.g. installation of a lift, tie downs, lowering the floor of the vehicle, raising the roof, etc.).

Remote Services

A form of assistive technology which includes a person-centered electronic network of devices enabling waiver participants to live more independently through support and monitoring while exercising more control over their own lives. Remote Supports are limited to: medication dispensers, door sensors, window sensors, stove sensors, water sensors, pressure pads, GPS Tracking Watches, and the remote monitoring equipment necessary to operate the remote supports technology.

Care, supervision, and skills training in a non-institutional setting.

Residential Habilitation - CTH I

Care, supervision, and skills training in a non-institutional setting.

Residential Habilitation - CTH II

Care, supervision, and skills training in a non-institutional setting.

Residential Habilitation - HASCI

Care, supervision, and skills training in a non-institutional setting.

Residential Habilitation - SLP I

Care, supervision, and skills training in a non-institutional setting.

Residential Habilitation - SLP II

Care, supervision, and skills training in a non-institutional setting.

Respite Care - Hourly

Care provided on a short-term basis because of the absence or need for relief of those persons normally providing the care in the participant's home.

Respite Care - Hourly - Statewide

Care provided on a short-term basis because of the absence or need for relief of those persons normally providing the care in the participant's home. This service is in the statewide respite program.

Specialized Medical Equipment, and Assistive Technology Assessment/Consultation

Specialized Medical Equipment, and Assistive Technology Assessment/Consultation- assessment of the specific needs related to the participant's disability for which specialized medical equipment and assistive technology will assist the participant to function more independently.

Support Center Services

Non-medical care, supervision and assistance provided in a non-institutional, group setting outside of the individual's home to people who because of their disability are unable to care for and supervise themselves.
